The purpose of this article is to present an engineering method for the determination of cause by the identification of defects that lead to failure. Further discussion on this topic is of course warranted and this article is anticipated to catalyze such discussions. In order to facilitate the development and presentation of this cause determination method, the definition of defect as it relates to failure is first presented.

Failure Analysis

In the present work, an analysis was carried out to know the wear modes present in connecting rod bearings from internal combustion engines. These mechanical elements were selected since they are exposed to different engineering failures such as incorrect assembly, severe loads, extreme temperatures, inadequate conditions, and loss of lubricity. Different techniques such as SEM and optical microscopy, EDS analysis, hardness testing, and surface profilometry were used to characterize the unworn and worn bearings. Wear mechanisms such as sliding wear scoring , fatigue wear with cracks where torn out material was clearly observed, discoloration areas, and two- and three-body abrasion wear rubbing marks were identified on the bearing surfaces. Engineering Failure Analysis. A connecting rod is a mechanical element that, subjected to tensile or compressive stresses, transmits the articulated movement to other parts of the machine.

Failure of a high pressure HP turbine blade in a military turbofan engine is investigated to determine the root cause of failure. Forensic and metallurgical investigations are carried out on the affected blades. The loss of coating and the presence of heavily oxidized intergranular fracture features including substrate material aging and airfoil curling in the trailing edge of a representative blade indicate that the coating is not providing adequate oxidation protection and the blade material substrate is not suitable for the application at hand. Coating spallation followed by substrate oxidation and aging leading to intergranular cracking and localized trailing edge curling is the root cause of the blade failure. The remaining portion of the blade fracture surface showed ductile overload features in the final failure. The damage observed in downstream components is due to secondary effects.

However, the pin bore of the combined piston is prone to deform leading to the failure of the piston. Based on the structure of the piston, the stress of the piston under thermomechanical coupling is analyzed, the temperature field of the piston is determined by experiments, and the deformation rule of the piston pin bore under the thermomechanical coupling is summarized. A design scheme is proposed to change the position of the thread connection between the piston crown and the piston head. Under the same conditions, the deformation of the piston pin bore of the original scheme and the new scheme is analyzed.
